Activity/Facilities Use Form

	POLICY: All areas & equipment used are to be left clean, with everything returned to where it was found, including furniture. Do not leave any food behind. All events/activities are to be completed, including cleanup, by 12 midnight. Decorating and setup activities are not to interfere with other scheduled events. All requests are on a first come, first served basis. If there is a conflict, you will be contacted by a church office staff member. Submission of this request indicates you have read and agree to comply with this policy.
